ALS changes what it needs every couple of months. Families usually start with a few hours a week for showers and end up somewhere they never pictured. Planning the next stage before it arrives matters more than it sounds, because finding good overnight coverage is much harder in a crisis than it is in advance.
Speech is often one of the first things ALS takes, and once it goes, a caregiver who already knows your husband can read him. Which sound means the pillow, which one means the straw. A stranger cannot do that, and starting over every week wears out the person who has to keep being understood. We keep the roster small and consistent on purpose.
Once a wheelchair, a hospital bed and a lift are in the house, moving somebody safely becomes a job done a dozen times a day. Our caregivers are trained in safe transfer technique, and we match you with people already comfortable around the equipment you have. Regular repositioning matters just as much, and it is the first thing to slide when a family is doing everything alone.
Swallowing gets complicated and mealtimes stretch out. Meals get made to the consistency your clinic dietitian has asked for, and the caregiver sits with your loved one rather than hovering, giving the meal the time it now takes. We follow the plan the clinic sets. We do not write it.
Nearly every ALS family hits the same wall, and it is the nights. Repositioning at two and four and six, a spouse who has not slept through in months, and a workday waiting in the morning. Awake overnight shifts and full 24-hour coverage are the two ways out of that. This is the single most common reason ALS families in Fort Myers and Sarasota call us.
There is usually one person doing most of this, and ALS burns that person out faster than almost anything else we see. Respite is not a luxury here. A caregiver in the house for six hours so a wife can go to her own appointment, sleep, or sit somewhere quiet is often what keeps the whole arrangement standing.
